Please fax referrals to +1 (229) 222-0668.  Include patient's full name, valid email address*, and phone number.  

Patients may self-schedule  using our booking link.   Registration forms and benefit coverage information will be sent to patient's email address after appointment is scheduled

*Valid email address is important in order for patients to receive the proper forms in a timely manner.  These must be completed within 72 business hours from the appointment date in order for benefit coverage information to be provided.
